I hereby acknowledge that I am the person described and I have read the following statements and do hereby release to Raymond C. Wisniewski, D.C., NutriMost, LLC, and the Offices and Businesses of Dr. Raymond C. Wisniewski, the rights to use my photograph and/or video and/or the written testimonial I have written, for the purpose of educational and/or advertising purposes.
I understand this testimonial and/or video/photograph shall be used on the various websites, advertising and/or for educational purposes, and may be re-used at future dates. Furthermore, I have accepted no fee or compensation for my participation. I hereby release any and all claims against NutriMost, LLC, Raymond C. Wisniewski, D.C. and his offices, businesses, and office personnel in connection with the use of my written testimonial and/or my video or photograph. I acknowledge that this agreement is final under these terms.
